Once you have registered with MDVIP and you have your scheduled appointment time please plan to arrive 15 minutes prior to your scheduled initial visit time. At that time you will need to complete the registration process. Please ensure you have a valid photo identification as well as your current health insurance card. If your plan requires you to have a primary care physician listed - please have our PPA providers name listed on the card.
It would be helpful to obtain the previous three years of your Medical Records from your previous provider for your initial visit. As well as a complete listing of current medications and vitamins or supplements you currently are taking.
The staff will be coordinating your comprehensive physical examination after your initial visit.
